| CASE MANAGER - HALIFAX Our client, the largest service provider in Canada for people with physical disabilities, is currently looking for an CASE MANAGER for its Halifax office. The role of the Case Manager is to provide services to adults who have disabilities and are challenged by employment barriers. These case management services include the direct delivery and coordination of intake assessment, service planning, vocational counseling, monitoring, job site analysis and job placement or development. Principal Job Responsibilities : Assessment and Service Planning: Coordinate initial assessment and assist the client by exploring vocational options so that he can select viable vocational goals ; Case Management: Develop en employment plan that details the services the client needs to overcome any employment barriers so that the client can achieve recommended goals ; Job Placement and Development: Coordinate internal OMOD services, especially job development and employment coaching to ensure the client receives needed services. Make referrals to external agencies when required ; Monitor client progress and develop solutions when the client experiences problems. Administration: Complete required reports and manage each case such that the client achieves their employment goal within the time frame and cost detailed in the service plan. Exigences : Bachelor`s degree in a related discipline - Members of the OCCOPPQ ( Ordre des conseillers et conseillères d'orientation et des psychoéducateurs et psychoéducatrices du Québec) or who have RRP (Registered Rehabilitation Professional)standing will be given preference; 2 to 4 years of experience in the delivery of job placement and vocational rehabilitation is preferred ; Excellent verbal and written communication skills. Ability to communicate in French and English is considered an asset ; Good organizational and problem-solving skills ; Effective negotiating skills ; Sensitivity to issues/barriers that people with disabilities encounter ; Computer skills in MS Word, Excel, Windows ; Thorough knowledge of local labour markets.
